Pharmacist's Advice
Clinical Associate Professor, University of Illinois at Chicago College of Pharmacy
Penicillin V comes with serious risks if you don’t take it as prescribed.

If you stop taking the drug suddenly or don’t take it at all

Your infection may not get better or may get worse. Even if you’re starting to feel better, you should still finish all of your medication.

If you miss doses or don’t take the drug on schedule

Your medication may not work as well or may stop working completely. For this drug to work well, a certain amount needs to be in your body at all times.

If you take too much

You could have dangerous levels of this drug in your body. Symptoms of an overdose of this drug can include:
  • Diarrhea
  • Nausea
  • Oral infection. Symptoms can include:
    • Creamy white lesions that look like cottage cheese
    • Cotton mouth (dry mouth)
  • Vomiting
  • Blackened tongue
If you think you’ve taken too much of this drug, call your doctor or local poison control center. If your symptoms are severe, call 9-1-1 or go to the nearest emergency room right away.

What to do if you miss a dose

Take your dose as soon as you remember. But if you remember just a few hours before your next scheduled dose, take only one dose. Never try to catch up by taking two doses at once. This could result in dangerous side effects.

How to tell if the drug is working

Your symptoms and your infection should get better.

Penicillin V is used for short-term treatment.

Penicillin V is the phenoxymethyl analog of penicillin G.
Penicillin V potassium is the potassium salt of penicillin V.

This discovery of the mold - Penicillium notatum - and his recognition of its special powers set the wheels in motion to create one of the most used drugs in medical history.
In March 1942, Anne Miller became the first civilian to be treated successfully with penicillin having almost died from a huge infection following a miscarriage.
Although Fleming often gets the accolade for having invented the first antibiotic, there was a lot of work to do before penicillin could become as commonly used and useful as it is today.
The bulk of the work was eventually carried out by scientists who had a much better-stocked laboratory and a deeper understanding of chemistry than Fleming. Dr. Howard Florey, Dr. Norman Heatley, and Dr. Ernst Chain carried out the first in-depth and focused studies.
Interestingly, and with impressive foresight, Fleming's Nobel Prize acceptance speech warned that the overuse of penicillin might, one day, lead to bacterial resistance.

How penicillin works

Penicillin works by indirectly bursting bacterial cell walls. It does this by acting directly on peptidoglycans, an important part of bacteria's structure.
The peptidoglycan in bacteria's cell walls increases their strength and keeps external fluids and particles from entering them.
Peptidoglycans form a mesh-like structure around the bacteria's plasma membrane.
When a bacterium multiplies, small holes open up in their cell walls as the cells divide. These holes are then backfilled with newly produced peptidoglycans, and the wall is reconstructed. That is unless penicillin is in the vicinity.
Penicillins inhibit the protein struts that link the peptidoglycans together in the wall. This inhibition prevents the bacteriumfrom closing the hole in its wall.
Because of the difference in pressure between the inside of the bacterium and the surrounding fluid, water rushes into the hole and the bacterium bursts.

How do bacteria become resistant to penicillin?

Contrary to popular opinion, it is not the patient who develops resistance to penicillin but the bacteria itself.
Bacteria have been around for billions of years. During this time, they have suffered extreme environments and, as such, have a great deal of experience in adapting. Additionally, they regenerate very rapidly, making any genetic changes across a population relatively swift.
There are three common ways in which bacteria can develop an immunity to penicillin:
  • Penicillinase - sometimes bacteria produce penicillinases (e.g. beta-lactamase), enzymes that degrade penicillin. This ability is then transmitted throughout the bacterial group via a plasmid (a small ring of DNA) in a process called conjugation - the bacterial equivalent of sexual reproduction, where new genetic information is shared between individuals.
  • Altered bacterial structure - some bacteria subtly change the format of their peptidoglycan wall or the penicillin-binding proteins so that the penicillin can no longer bind to it.
  • Penicillin removal - other bacteria develop systems to export penicillin. Bacteria have efflux pumps that are used to transport substances out of the cell. Some of these pumps can be repurposed to dispose of penicillin.

Penicillin side effects

Below are some of the side effects that have been noted during penicillin usage:

Common side effects

  • Less common side effects
Vaginal itching and discharge - due to either a yeast infection or bacterial vaginosis
  • Sore mouth and tongue, sometimes with white patches
  • Diarrhea
  • Headache

  • Shortness of breath or irregular breathing
  • Joint pain
  • Sudden lightheadedness and fainting
  • Puffiness and redness of the face
  • Rashes and hives
  • Scaly, red skin

Rare side effects

  • Anxiety, fear, or confusion
  • Sense of impending death
  • Anxiety and hallucinations
  • Yellowing of the eyes and skin
  • Sore throat
  • Unusual bleeding
  • Diarrhea and reduced urination
  • Convulsions
  • Abdominal cramps, spasms, tenderness, or pain
  • Nausea and vomiting

Potential problems with penicillin use

Although penicillins are widely used, as with any drug, there can be some issues or contraindications that occur:
  • Breast-feeding - breast-feeding mothers may pass small amounts of penicillin to their babies. This can result in allergic reactions, diarrhea, fungal infections, and skin rash.
  • Interactions - some drugs can interact with penicillin. It is important to check with a doctor before taking multiple medications.
  • Bleeding problems - some penicillins (carbenicillin, piperacillin, and ticarcillin) can make pre-existing bleeding problems worse.
  • Oral contraceptives - penicillins can impede the effectiveness of oral contraceptives, raising the chances of pregnancy.
  • Cystic fibrosis - patients with cystic fibrosis are more prone to getting fever and skin rash when taking piperacillin.
  • Kidney disease - patients with kidney disease have an increased risk of side effects.
  • Methotrexate - methotrexate interferes with cell growth and is used to treat a number of conditions including leukemia and some autoimmune problems. Penicillin prevents the body from disposing of the drug, potentially leading to serious complications.
  • Phenylketonuria - some stronger, chewable amoxicillin tablets have high levels of aspartame that the body converts to phenylalanine. This is dangerous for anyone with phenylketonuria.
  • Gastrointestinal problems - patients with a history of stomach ulcers or other intestinal diseases might be more likely to develop colitis when taking penicillin.

Penicillin allergy

Some people exhibit allergies to penicillin. Allergic reactions normally include hives, wheezing, and swelling, particularly of the face. Around 10 percent of people report an allergy to penicillin but the true figure is closer to 1 percent, and only 0.03 percent exhibit life-threatening allergic effects.

Ex-miner recalls late leader's 1964 speech in Germany

This is the third in a series of articles highlighting Korean miners and nurses who went to Germany in the 1960s and 70s. — ED.

By Kang Hyun-kyung

Prof. Kwon Yi-chong
The late President Park Chung-hee's emotional speech in the German city of Duisburg on Dec. 10, 1964, pulled at the heart strings of a group of Korean miners who were there, far from home.

Kwon Yi-chong, one of some 400 miners present at the speech, recalled during a recent interview that almost all of those who gathered there wept during Park's speech.

"In fact, we, miners, became emotional from when President Park entered the hall to deliver his speech. When he approached the podium, I began sobbing as I couldn't control my emotions," Kwon said.

"President Park and first lady Yuk Young-soo couldn't leave the hall for hours as weeping miners asked them to stay longer," he said.

Kwon, now a professor emeritus at the Korea National University of Education, said hundreds of miners, who were immensely homesick, traveled by bus to meet Park in person.

"Looking at your tanned faces, my heart is broken. All of you are risking your lives every day as you go down thousands of meters underground to make ends meet," Kwon remembered Park saying. "What poor people you are! You go through these trying times just because Korea is so impoverished."

Park's emotional speech went on for a while.

"Although we are undergoing this trying time, we are not supposed to pass poverty onto our descendents. We must do our part to end poverty in Korea so that the next generation doesn't experience what we are going through now," Park noted.

Park, father of incumbent President Park Geun-hye, pointed out the root causes of poverty in Korea, comparing it with Germany.

"About 150 years ago, the industrial revolution was in full swing in Germany, whereas Koreans had no idea of how the world outside was changing. Koreans stuck to their traditional way of life without knowing what was going on outside the country. We were like frogs in a well," Park said.

"How can a country like Korea, which was not fully prepared for the upcoming era, be as rich as Germany now?"

Kwon went to Germany months earlier prior to Park visited Duisburg.

He said Korean miners experienced a number of challenges, including the language barrier, as they did the precarious job in Germany.

Despite this, he said, he was able to make his German dream come true thanks to several kind-hearted Germans he met.

"Life was rugged while I was in Germany for 16 years. I went there as a coal miner in 1964, years after I graduated from high school and extended my stay there as I pursued degrees in education," he said.

"Life didn't turn out the way that I expected. After my three-year work contract ended, I was to return to Korea. I packed all my belongings and went to the airport to come back. But a German lady who took care of me just like a godmother there asked me to stay in Germany as she knew that I longed for a degree there."

It took 16 years for Kwon to make his German dream come true.

"Studying at a German university in a foreign language was tough. Besides the language barrier, I had to deal with another formidable challenge _ poverty. It was hard to make ends meet there. So I cried in the rest room whenever I felt I was overwhelmed by the daunting challenges facing me," he said.

Kwon's hard work paid off as he eventually earned a Ph.D. in education. He returned to Korea in 1979 and worked as a professor at several different universities.

I. The Political Background

On December 22nd, 1963, the first group of 123 South Korean miners landed on West German soil. For the next 20 years, some 8,600 South Korean miners and 10,400 nurses followed (1). The miners were hired by German mines under contracts of the West German government and Korean embassy. Before coming to Germany, most of the miners had to pass health checks and interviews held by Korean government. Competition was fierce; in 1963, 375 men out of 2894 who applied got the job. Many of them were high school or college graduates, ranging from real miners to bankrupt entrepreneurs and high school teachers. So great was the public attention that the list of the names was even posted on daily newspapers (2). The to-be-miners were educated of basic mining skills and some German prior to their flight

The cause of such massive, government-led labor export lay in the economic circumstances of both countries. On one hand, in West Germany the Berlin Wall was built in 1961, cutting off the supply of fresh labor from East Germany (3). Japan, which had been sending 400 miners to West Germany annually since the 1950s, had stopped its labor export in 1963 (4). The danger and low payment of the job contributed to the low supply of mining labor within West Germany; more labor was essential in maintaining its growing economy.
On the other hand, the South Korean economy after the Korean War had been devastated: the per capita annual income was only US $ 87 in 1961 (5). In 1961, then Major-General Park Chung-hee led a military Coup d'etat and became a president in 1961, issuing the First Five Year Plan (1961) to establish an independent economy. However, the US was reluctant to help his military junta, lest it may lose its democratic foothold in East Asia (6). President Kennedy stopped US gratuitous aids, and smaller loans came instead; this was a strong blow for the Korean economy heavily dependent on US aids. Park's plans, and his hard-earned regime, were to collapse without monetary source from outside. They turned to West Germany for dollars. According to many Korean newspapers, it is widely believed that Korea could borrow 150 million Marks in return to Park's making a "heroic" gesture of sending workers to West Germany. It has become a legend that when Park and his wife visited the miners in Germany and made a speech on "how deeply sorry he was for the poverty of the fatherland", all the miners, as well as Park and Heinrich Lübke, then German president, "cried while singing the national anthem of Korea." (7) However, some argue that the loan was unrelated to the export of miners, and that such false reports were made just to "bring about Park syndrome." (8) Indeed, due to the repressed characteristics of Korean media in 1960s, there seems to be no official record on exactly how the loan was related to the export of miners; the remarks on Park's presidency still vary from sheer criticism to chauvinistic appraisal.

II. The Korean Miners' Experiences in Germany

Whatever political reasons may lurk behind the scenes, most Korean miners worked hard to accommodate. Many successfully overcame the barrier of language, culture and food while some, unable to assimilate to the harsh work environment, fled to other countries or even committed suicide (9). Most of the miners' incomes were sent back to Korea for their families. Realizing that German social security system gave merits to those married, many of them started to date Korean nurses on weekends.
After the 3-year-mining contract had expired, many miners remained in Germany and took other jobs such as sailors or shop-owners. Some went to German universities, benefiting from the relatively cheap education system. A minority married German women. In 1973 they formed "Glück Auf", an association for miners who remained in Germany : "Glück Auf" means "Good Luck", a salute for miners (10).
In 1967, "Dongbaengnim," or the "East Berlin Spy Incident," took place. From West Germany and France, the South Korean CIA "forcefully repatriated," or abducted, some 30 alleged spies for North Korea, including three miners; their charge was being involved in a network controlled by East Germany (11). Two of them were first charged with death penalty, then on life imprisonment, while many pleaded that they "had cooperated only to get news of relatives in North Korea." Their "spying" charge is still largely contested; some later confessed to have falsely admitted the charge due to torture and threats to the family remaining in Korea (12). The relationship between South Korean and West German government chilled quickly over this "gross violation of sovereignty in the original arrests"; this may have affected the gradual decrease of labor export in the 1970s (13).

III. The Termination of the Immigration of Korean Miners to Germany

In 1970s, as the first oil shock hit Europe, West Germany's economy stagnated. With high unemployment rates, German miners began to return to their jobs, causing less demand for "Gastarbeiter", or guest workers (14). Half a million Gastarbeiters had been deported to provide more jobs for Germans (15); some deportations of Korean nurses were met by protests by Koreans migrants (16). The last team of Korean miners arrived in Germany in October 25th, 1977; in 1978, West German government prohibited all new employments of Gastarbeiters (17). .
In conclusion, many Korean miners seem to have worked with not only their strong will to succeed, but also their sense of nationality. The money they sent was essential to their poverty-stricken country and their families; they were part of the "miracle of the Han river." Although there exist numerous contests on what political purpose they had been sent to, as well as attempts to overly dramatize them for political purposes, one cannot deny the fact that those individuals, now very much all retired, deserve a page in history.

If one knows our harmony, then he: Anton Hübler, who worked for Eintracht from 1954 to 1995, is one of the most intimate experts of the association. And so it is not surprising that there were always requests, whether he would not even write a book about his experiences at the Riederwald. But Anton Hübler beckons. "I do not carry anything into the cabin and nothing out of it," he once said in an interview, and Toni is still holding on to this day. This loyalty to the club, the players and the public is a result of Toni's life and is one of the reasons for his popularity.
In 1954 Toni Hübler came to Riederwald by chance. The learned gardener, who lived in Urberach, wanted to introduce himself to Eintracht. And ran from the main station straight to the Waldstadion. Here he was informed that the Eintracht was located at Riederwald. So it went on foot again through the city. With a few hours delay Hübler then appeared for the interview - and received an eight-week trial contract. However, he was already changed to an indefinite contract of employment after six weeks.
From then on, Hübler worked as a gardener at the plant erected two years earlier. He planted the area with poplars, who needed a lot of water and thus kept the ground water. He took care of the playing fields, helped with tournaments from the footballers to the rugby players - and was often up late at night. The then President, Rudi Gramlich, appreciated the reliable staff, and Toni was also chauffeur of the President. With the introduction of the Bundesliga he became a witness, from then on he took care of the interests of the League players of Eintracht. It was not just about shoes, jerseys and balls - Toni Hübler was always the "grief box" for the players. With good tips, encouraging words and guaranteed secrecy. And sometimes he was also an interpreter. Toni, who was born in the Hungarian Pecs (Five Churches), speaks perfectly Hungarian and was able to translate for Istvan Sztani in the 1950s and for Lajos Detari in the 1980s.
Toni Hübler traveled through the half world with the concord, he experienced four cups and the Uefa Cup victory. There were some low points, too, and sometimes the nerves had to be spared. So he spent the second half of the "descent game" against the OFC in the winter of 1984 in the cabin. "I was too excited to even have the showers, so I do not hear," he laughs. The harmony with Toni has never subsided. In 1984 the rescue succeeded in the relegation against Duisburg, 1989 against Saarbrücken. Actually, it was only logical that something had to go wrong when Anton Hübler went to the well-earned retirement in 1995. A year later, Eintracht was only second. To this day, Toni Hübler accompanies his concord with much interest. He plays in the stadium every time he plays. The enthusiastic hunter is still regularly on the road, and almost 20 years after his professional departure from the Riederwald, he still has his own fan club with the "EFC command Anton Hübler". Once a year his wife Christel and he invite the fans to Urberach, then on the terrace or in the legendary cellar bar celebrated. At the end of April there was a lot going on at the Hüblers: on April 28, Toni Hübler, the good soul of Eintracht, celebrated his 85th birthday. Actually planned only in small circle, with family and neighbors. Helga and Friedel Lutz, Bernd Nickel, Bernd Hölzenbein and Eintracht printer Peter Wuschek came nevertheless, but they belong to the family. And all the rest of the Eintracht family congratulate the Toni in this way with all your heart!
